Mountain Valley promotes good health by scheduling regular doctor's appointments and follow-up care.
Our on-site Registered Nurses assure professional support and coordination of medical services.
Case Management Services:
Each person who receives services from
Mountain Valley will have a case manager to
advocate for their needs and assist them with
various aspects of their lives.
- Locating and applying for desired services and placements
- Acquiring and maintaining benefits and supports from other agencies.
- Developing and monitoring treatment programs to meet the needs of persons receiving services.
- Individuals with a disability which originated between birth and 21 years of age, which constitutes a substantial handicap attributable to mental retardation or related conditions.
- Children from birth to five years of age who do not meet the above criteria, but exhibit delays in development or have a history of birth complication, postnatal trauma or illness.
